The initial information required includes:
- Subcontractor company name and representative
- Description of plant/equipment
- Make and model
- Serial number
- Year of manufacture
This information helps in identifying and documenting the specific crane being inducted.
The following documentation and records should be available:
- Plant hazard risk assessment
- Manufacturer's operator manual
- Preoperational check log book
- Current maintenance records (refer to operator's manual for service intervals)
- Registration of plant design information (required if lifting capacity is greater than 10 tonnes)
- Compliance/ID plates
- Ten-year certification (major inspection completed if applicable, then every five years)
- 12-month annual inspections (current Green Sticker)
These documents ensure that the crane is properly maintained and safe for operation.
Specific safety checks for a crane include verifying:
- Load charts fitted in the cabin
- Reverse beeper working (for non-slewing crane)
- Flashing light working (for non-slewing crane)
- All lifting chains fitted with tags identifying SWLs, serial numbers, and test dates
- All other lifting equipment marked with SWLs, serial numbers, and test dates
- Fire extinguisher fitted with a current test tag (for cabin operated cranes)
These checks ensure the crane meets safety standards and is equipped with necessary safety features.
The person completing the form needs to sign off on it. This includes their name and signature. The sign-off serves as a formal acknowledgment that all necessary checks and documentation are in place and that the crane is ready for operation on site.
