The initial information required includes:
- Subcontractor company name and representative
- Description of plant/equipment
- Make and model
- Serial number
This information helps in identifying and documenting the specific earthmoving equipment being inducted.
The following documentation and records should be available:
- Plant hazard risk assessment
- Manufacturer's operator manual
- Preoperational check log book
- Current maintenance records (refer to operator's manual for service intervals)
These documents ensure that the earthmoving equipment is properly maintained and safe for operation.
Specific safety checks for earthmoving equipment include verifying:
- Roll Over Protection System (ROPS) and Falling Object Protective Structure (FOPS) are fitted
- Operator restraint (seatbelt or locking-bar) is fitted
- Seatbelt is within one year of plant manufacture date or two years of installation date (CAT specific equipment)
- Flashing light is working
- Reverse beeper is working (if required)
- Safe working loads (SWL) are clearly marked and load chart is displayed (if lifting)
- Burst protection valves are fitted (required for equipment with a rated capacity over one tonne)
- All lifting chains are fitted with tags identifying SWLs, serial numbers, and test dates
- All other lifting equipment has SWLs, serial numbers, and test dates clearly marked
- Equipment is road registered (if being driven on public roads)
These checks ensure the earthmoving equipment meets safety standards and is equipped with necessary safety features.
The person completing the form needs to sign off on it. This includes their name and signature. The sign-off serves as a formal acknowledgment that all necessary checks and documentation are in place and that the earthmoving equipment is ready for operation on site.
