The initial information required includes:
- Subcontractor company name and representative
- Description of plant/equipment
- Make and model
- Serial number
- Year of manufacture
This information helps in identifying and documenting the specific EWP being inducted.
The following documentation and records should be available:
- Plant hazard risk assessment
- Manufacturer's operator manual
- Preoperational check log book
- Current maintenance records (refer to operator's manual for service intervals)
- Registration of plant design information (Design Registration No.)
- Ten-year certification (major inspection completed if applicable, then every five years)
These documents ensure that the EWP is properly maintained and safe for operation.
Specific safety checks for an EWP include verifying:
- Motion beeper is working
- Flashing light is working
- Gate locks are engaging and self-closing gate is operational
- Platform handrail/frame is in good condition
- Equipment is electrically tested and tagged (RCD & charging cables, if applicable)
- Safe working load (SWL) of the platform is clearly marked
- Safety harness is in good condition with inspection documents available (for boom type MEWP)
- Secondary guarding (safety crush) device is fitted to the elevated work platform
- Emergency stop and ground control functions are checked
These checks ensure the EWP meets safety standards and is equipped with necessary safety
The person completing the form needs to sign off on it. This includes their name and signature. The sign-off serves as a formal acknowledgment that all necessary checks and documentation are in place and that the EWP is ready for operation on site.
